Stichting Gezondheidscentra Haarlemmermeer (hereinafter: SGH ) considers careful handling of personal data to be of great importance. In this privacy statement, we describe which personal data we process from you, how we handle your personal data and what rights are to be disclosed to you in that context.
SGH is responsible for data processing. In this privacy statement we explain what personal data we collect and use and for what purpose. We recommend that you read it carefully. In our processing, we comply with the requirements of privacy legislation. This means, among other things, that:
- we clearly state for what purposes we process personal data. We do this through this privacy statement;
- we limit our collection of personal data to only the personal data required for legitimate purposes;

Personal data and purposes
The self-triage service “Should I go to the doctor?”
When using the self-triage service “Should I go to the doctor?” (below: App), via our website you fill in certain personal data and leave certain personal data. Because it is privacy-sensitive data, we think it is important that we handle it carefully.
When using the App, you fill in personal data about yourself or about someone else for whom you use the App and a conclusion is given. The personal data we process from you through this service are:
- Genus
- Date of birth
- Your GP
- Outcome of each completed triage (urgency)
In all cases, the data you provide is personal data. An example of this is the fact that the App creates a Unique Identifier (UID for the device used). Nevertheless, we would like to inform you clearly about what we do with your data. The completed data is initially necessary to answer the question ‘should I go to the doctor?’. Your gender and date of birth will be processed anonymously by the app’s back-end to analyze the use of the App. It can then ensure that the App works even better in the future and we can generate anonymous statistics. Your IP address is processed by the web server anonymised into a session number, which makes your personal data untraceable.
Your location is stored only on the device you use the website with and if you use the link to a map application (such as Google Maps) to get directions directly to your GP. This data is not stored by us, but by the supplier of the card application. We recommend that you read the relevant privacy statement.
Watch out. If you have questions about the outcome of the app’s self-triage or whether you should go to the doctor, please contact your GP or gp post. You can only make reports of incorrect or incomplete data via the contact form and you can also request technical support. If you provide feedback about the App, you are not supposed to send medical information related to yourself.
Feedback
We invite you to leave feedback for us. When you respond to this invitation, we will use your feedback, in conjunction with your gender and date of birth, to improve our service. We will keep your feedback for up to two years after you submit it.
Who do we share personal data with?

others Your personal data is not shared with third parties for commercial purposes. In SGH addition, third parties may engage in the performance of its services. For example, the App is developed and hosted by a third party. Third parties SGH are processors of .
